Patrick Dean Hill, APRN, CNP
Dean was born and raised in Collinsville, Texas. He moved to Madill Oklahoma in 1995 and is proud to call Oklahoma home. He received his LVN from Grayson County College in 1995 and his Associate degree in Nursing in 1995. He went on to earn his Bachelors in Nursing from East Central University in 2000 and furthered his education with a Masters Degree from Texas Women’s University in 2005. Dean brings a vast amount of knowledge in family medicine, geriatrics and emergency care.
He is a member of the American Association of Nurse Practitioners, the American Nurses Credentialing Center, a subsidiary of the American Nurses Association and is actively involved with special Olympics and animal rescue.
In his spare time, he enjoys fishing, boating, spending time with family and hunting zombies with his grandson.
Dr. Michael W. Carnahan, M.D.
Dr. Michael W Carnahan, M.D., a Board Certified Diplomat in Family Practice, was born and raised in Lawton, Oklahoma. After earning a Bachelor of Science degree at The University of Oklahoma, Dr. Carnahan attended the University of Oklahoma College of Medicine where he received his Doctor of Medicine Degree in 1986. In 1989, he completed his residency at St. Joseph Hospital in Wichita Kansas. That same year, he moved to Ardmore and joined Family Health Center.
Dr. Carnahan has over 28 years experience in primary care including but not limited to geriatric, adult, pediatric and occupational medicine. He is the Medical Director of Mercy Rehabilitation Center, the Director of skilled and long term care units at Elmbrook and Southbrook nursing facilities. He helped found the Good Shepherd Community Clinic in 1996 which has grown from a small volunteer clinic treating just 85 patients to a full time innovative medical facility caring for more than 3,000 socio-economically disadvantaged patients annually and actively serves on the Board of Directors. Dr. Carnahan is the co owner and supervising physician of Family Practice Associates who does business as Family Health Center and Family Health Convenient Care Clinic. He holds a professional membership with The American Medical Association and The Oklahoma State Medical Association.
Dr. Carnahan enjoys golf, hiking, racquetball, spending time with his family and has been on multiple medical mission trips, including Uganda Africa. He is very involved in Church and teaches on a regular basis.
Dr. Thomas W. McCulloh, D.O.
Dr. Thomas W. McCulloh, an Ardmore Oklahoma native, graduated from The University of Oklahoma in1972 and earned his medical degree from Oklahoma State University College of Osteopathic Medicine in 1977. He completed his Internship at Garden City Hospital in Garden City, Michigan and completed his residency in 1983 at Wayne State University in Michigan. He is the co owner and supervising physician of Family Practice Associates who does business as Family Health Center and Family Health Convenient Care Clinic, which he founded in 1983. Dr. McCulloh also founded the Good Shepherd Community Clinic in 1996 which has grown from a small volunteer clinic treating just 85 patients to a full time innovative medical facility caring for more than 3,000 socio-economically disadvantaged patients annually. He is the Director of Skilled and Long term care units at Elmbrook and Southbrook Nursing facilities, Rehab Physician of Mercy Rehabilitation Center since 2010 as well as Adjunct Clinical Professor of Family Medicine Oklahoma State University College of Osteopathic medicine. He is Board Certified in Family Practice with added qualifications in Geriatric Medicine by the American Osteopathic Board of Family Physicians and was awarded Doctor of the Year in 2008 by the Oklahoma Osteopathic Association.
Dr. McCulloh believes in the importance of working as a team with patients and families to provide the best health care possible. In his spare time, he enjoys doing home improvements and spending time with his family and dog Millie.

Laurel McCulloh, APRN, CNP
Laurel received her Master of Education Degree from the University of Alabama, Bachelor of Science in Nursing from Edinboro University, Master of Science in Nursing from the University of Alabama and Post Masters Family Nurse Practitioner Degree from Texas Womens University.
Laurel has been in practice for thirty seven years and is an active member of the American Association of Nurse Practitioners, The National Association of Pediatric Nurses and Practitioners and Urgent Care Association of America.
Laurel was born and raised in Pennsylvania. In her spare time, Laurel enjoys reading, exercise, gardening, cooking, knitting, crocheting & sewing.
